The purpose of this case study is to improvement of Korean medical treatment and evaluate prognosis and clinical course of ALS patients. Methods : The patient was treated by acupuncture, Su-Gi therapy, herbal medication, physical treatment. To determine the effects of Korean medical treatment, we evaluated every four days used by Korean version of Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (K-ALSFRS-R), Amyotrophic Lateral Sclerosis Severity Scale(ALSSS). Results : Before mental contraction caused by everyday life nursing management, symptoms such as lumbar pain, getting hoarse, lower limb powerless & spasticity, upper limbs powerless, neckache, tongue-tied etc. were improved after above treatment. K-ALSFRS-R and ALSSS scores were also increased till patient’s mental contraction Conclusions : The Korean medical treatment is effective on local symptoms of ALS, and there is also functional improvement of ALS diagnosed by external examination. NRS (Numerical Rating Scale) Ⅲ. 증례 1. 초진 사항 1) 환자 2) 주소증 3) 발병일 4) 가족력 5) 과거력 6) 현병력 7) 초진소견 2. 검사 소견 1) 방사선 검사 2) 임상병리 검사 3) 경락 기능 검사 3. 치료 방법 1) 침구 치료 2) 물리 치료 3) 醫療氣功 : 導引按蹻法 4) 한약 치료 5) 양약치료 4. 치료 경과 1) 기간별 경과 기록 2) 평가 결과 Ⅳ. 考察 Ⅴ. 結論 Ⅵ. 參考文獻 --------- [서론] 근위축성 측삭경화증(amyotrophic lateral sclerosis, ALS)은 운동신경세포에 퇴행성 변화가 생겨 근육 쇠약 및 위축을 일으키는 질환이다[1]. 증상은 감각 기능에는 전혀 이상 없이 사지 근력약화, 강직, 위축, 속상수축 등의 운동성장애, 연하∙저작장애, 언어장애, 호흡장애 등을 보이고[2] 종국에는 호흡근 마비로 사망한다[1]. 증상 발생 후 3~5년 이내에 사망하는 치명적인 질환임에도, 이에 대한 뚜렷한 원인과 치료법이 없고, 증상의 완화를 목표로 하는 대증요법(symptomatic treatment)과 각종 부작용과 합병증을 조절하고 완화시키기 위한 지지요법(supportive therapy)으로 환자를 관리하는 것이 현 치료 경향이다[3]. 병의 빠른 경과와 임상양상의 다양성으로 인해서 초기 진단이 어렵고, 인구 10만 명당 4~6명의 낮은 유병룔(1~3명의 발병룔)로 연구의 대상자가 적고, 임상에서 서로 다른 진단기준과 연구방법이 사용됨에 따라 표준화되고 체계적인 치료가 진행되지 않고 빠른 진단과 예후 예측이 되지 않고 있는 실정이다[4]. 그러므로 차후 ALS 치료에 있어서, 환자 상태를 객관적으로 정량화하여, 예후를 검증하고 질환의 진행속도 및 양상을 파악하고 조기에 치료하는 것이 필요하다[1]. 국내 ALS 역학연구를 위해 ALS 및 운동신경세포질환(motor neuron disease, MND) 환자를 대상으로 다기관 등록체계(registry)[4]를 구축하여 체계적인 관리 및 향후 치료지침 개발, 그리고 표준화를 이루려는 국내의 움직임이 있고, 그 일환으로 Korean ALS Functional Rating Scale - Revised(이하 K-ALSFRS-R)지표를 구성하여 수치를 정량화함으로써 예후를 예측하고, 진단이 늦어져서 집중치료에 돌입하는 시기가 지연되는 일을 방지하고자 하였다[4]. 반면에 ALS에 대한 한의학적 임상연구는 일정한 효과에도 불구하고 체계화되지 못한 문제가 있어 보완이 필요하다. 이에 저자는 2019년 04월부터 06월까지 김포 A 한방병원에 입원한 ALS 환자 단일 사례를 K-ALSFRS-R을 중심으로 평가하여 그 결과를 보고하는 바이다. 덧말
