Methods : The patient was administered with additional Gagampyeongwi-san and Sinseonbulchuidan for 30 days. For acupuncture, the colonic reducing method (大腸勝格, LI-) and spleen tonifying method (脾正格, SP+) of hand acupuncture were applied. 內關 (PC6), 公孫 (SP4), 三陰交 (SP6), and 足三里 (ST36) were basically collected, and additional 率谷 (GB8), 合谷 (LI4), and 百會 (GV20) were additionally drained and collected by body acupuncture. During the inpatient treatment period, three liver function tests were performed. Results : AST and ALT were restored to the normal range, and γ-GTP and CPK were also reduced, indicating that alcoholic hepatitis was greatly recovered. Total bilirubin and direct bilirubin were also recovered close to normal, and it was predictable that the liver excretion capacity due to hepatocellular damage was also restored, and the findings of biliary obstruction were alleviated close to normal values. When looking at the rise of eGFR along with BUN and uric acid, it seems that kidney function was recovered and improved. Conclusions : Through the combination treatment of Korean Medicine and fluid treatment, it was effective in recovering jaundice and eating disorder for alcoholic liver disease. * Keywords : Alcoholic liver disease, Jaundice, Eating disorder, Gagampyeongwi-san, Sinseonbulchuidan [목차] Ⅰ. 緖論 Ⅱ. 症例 Ⅲ. 考察 Ⅳ. 結論 Ⅴ. 參考文獻 [서론] 알코올성 간질환(Alcoholic Liver Disease : ALD)은 초기 단계의 지방간에서 알코올성 간염, 간경화를 거쳐 간암[1], 간부전을 발생시킬 수 있다. 습관성 음주자의 대부분(90~100%)이 가지고 있는 알코올성 지방간은 금주하면 어느정도 가볍게 정상으로 회복 가능하나, 10~35%의 알코올성 간염, 8~20%의 알코올성 간경변증[2]은 각각 급성 알코올성 간염의 사망률 30~60%[3], 간경화의 사망률 22%[4], 황달,복수 등의 합병증[5] 시 5년사망률 58-85%, 간암 5년생존율 39.3%[6] 수준으로 사망률이 높고 금주하더라도 임상경과가 악화될 수 있어 치료 시 금주를 필수로 하는 것 외에도 조기에 적절한 의학적인 조치를 통하여 비가역적인 간 손상으로의 악화를 예방하는 것이 중요하다. 특히, 세계보건기구에서 추산한 2020년 15세 이상 한국인의 1인당 연간 알코올 소비량은 7.9 L로 OECD 평균인 8.4 L보다 적다[7]. 하지만 주류의 도수가 낮아지면서 국내 1인당 알코올 소비량은 소폭 감소하고 있으나, 고위험 음주와 관대한 음주문화, 섭취량(1회 음주량)의 증가, 여성 음주율의 증가로 음주의 위험성이 증가하고 더욱이 알코올성 간질환의 발병가능성은 여전히 높다고 볼 수 있다. 한약에 대한 간독성 여부는 오래된 논란 거리였다[8-10]. 물론 세상의 모든 약은 간에 스트레스를 줄 수밖에 없지만, 한약의 간독성이 양약에 비해서 훨씬 덜하다는 여러 연구[11-14]와 오히려 간질환을 치료한다는 연구[15-17]가 지속적으로 발표되고 있음에도 불구하고 일부 왜곡된 양의계 인사들이 지속적으로 한약과 한의학을 폄훼하고자 한약의 간독성 문제를 침소봉대하여 제기하는 것은 국민들의 올바른 의료선택권을 방해하는 중대한 잘못이라 할 수 있다. 환자를 치료함에 있어 한방과 양방이 따로 있을 수 없고 오직 환자의 건강회복만을 위해 적절한 타이밍에 적절한 치료를 받을 수 있도록 의료인은 최선을 다해야 한다. 이에 저자는 황달과 섭식장애를 주소증으로 하는 알코올성 간질환 환자를 치료하여 심각한 간기능 수치와 호소증상이 회복된 사례가 있어 보고하고자 한다. 덧말
