However, taking hormone medication does not improve clinical symptoms or side effects, so it is necessary to develop other improved treatments. The purpose of this study is to report that the Korean medicine treatment along with Whidam’s Gokgol(CV2) Sugi therapy has improved symptoms of hypothyroidism and related the level of TSH, free T4 after halting the use of levothyroxine. Methods : We employed Korean medicine treatment; herbal-medication(延齡固本丹 and 加減平胃散), acupuncture, and cupping therapy, and so forth. At the same time, we treated the patient with Whidam's Sugi therapy(especially, Gokgol Sugi therapy). We treated the patient more than five times a week for the first month and treated more than three times a week for the next three months. Since then, we have treated the patient once or twice a week. The patient took a total of four blood tests during her treatment in 2012 and 2017. Results : 1. Both TSH and free T4 levels and clinical symptoms were normalized for patient with hypothyroidism who stopped taking levothyroxine by treating the patient with Korean medicine treatment and Whidam's Sugi therapy. 2. The combination of Korean medicine treatment and Whidam's Sugi therapy was also used in the case of subclinical hypothyroidism patient. 3. The combination of Korean medicine treatment and Whidam's Sugi therapy for hypothyroidism lasted for five years, not just for a short period of time. Conclusions : The combination of Korean medicine treatment and Whidam's Sugi therapy(especially, Gokgol Sugi therapy) was effective in the treatment of hypothyroidism and it helped to normalize the level of TSH, free T4. It is hoped that further clinical research will continue in the future so that it can be used not only for hypothyroidism but also for other endocrine diseases. * Keywords : hypothyroidism, Whidam’s Gokgol(CV2) Sugi therapy, Korean medicine, TSH, free T4, Levothyroxine, 갑상선기능저하증, 휘담식 곡골수기, [목차] Ⅰ. 緖論 Ⅱ. 本論 1. 증례 기본 사항 1) 예진 사항 2) 초진 사항 2. 치료방법 1) 침치료 2) 약물요법 3) 부항요법 4) 온열요법 5) 휘담식 수기요법 3. 임상 경과 Ⅲ. 考察 Ⅳ. 結論 Ⅴ. 參考文獻 [서론] 갑상선기능저하증은 갑상선 호르몬 생성의 감소로 인하여 나타나는 일련의 증상을 총칭하는 증후군으로서 임상적으로 나타나는 가장 흔한 갑상선 기능의 이상이다[1]. 통계청에 따르면 갑상선기능저하증 관련 한양방 합산 건강보험공단 급여 지출 총액은 2013년도에 비해 2018년도에 1.79배 상승한 것으로 나타났다. 하지만 아직 상대적으로 한방에선 갑상선기능저하증 관련 치료 건수가 미미한 실정이다. 갑상선기능저하증의 경우 갑상선 호르몬 결핍으로 인해 발현되는 증상 정도는 다양하나 피로, 체중증가, 한불내성, 변비, 피부건조, 월경불순 등의 공통적인 임상증상을 보인다[2]. 갑상선기능저하증 치료의 목표는 위와 같은 임상증상의 진행을 막고, 정상적인 호르몬(TSH, T4) 수준을 유지시켜 정상적인 대사과정이 진행되도록 하는 것이다[3]. 양방에서는 주로 갑상선호르몬제인 Levothyroxine (L-Thyroxine)를 투여하여 부족한 갑상선호르몬을 보충하는 호르몬요법을 사용하고 있으나, 장기간 복용해야 하는 불편함, 허혈성 심장질환의 발생, 갑상선 호르몬 요구량의 변화 등의 문제점과 함께 갑상선호르몬을 보충해주더라도 임상증상이 개선되지 않는 문제점도 발생하고 있다[4]. 따라서 좀 더 근본적인 치료를 하고 부작용이 적은 새로운 치료법이나 치료제의 개발이 필요한 실정이다. 한의학에서 갑상선 기능 저하증은 浮腫, 虛勞, 行遲, 語遲, 結陽證 등의 범주에 속한다고 보고 있으며, 그 원인에 대해 살펴보면 氣血不足, 脾腎陽虛, 命門火衰, 心腎陽虛 등으로 나눌 수 있으며 補氣補血, 溫補脾腎, 溫補腎陽, 溫補心陽 등의 치법을 통해 치료한다[5]. 갑상선기능저하증에 대한 한의학의 연구도 다수 이뤄지고 있는데, 대부분은 실험적 연구로 2000년이후 발표된 갑상선기능저하증에 대한 실험적 연구를 살펴보면 본초 단미제 약물로는 乾薑[2], 金銀花[6], 附子[7], 柴胡[8], 吳茱萸[9], 烏梅[10], 肉蓯蓉[11], 淫羊藿[12], 人蔘[13], 菟絲子[14], 巴戟天[4], 黃芪[15] 등이 있으며 한약복합처방으로는 甘芎湯[16], 當歸四逆湯[17], 大營煎[18], 右歸飮[19], 益氣補血湯[20], 人蔘養榮湯[21], 滋陰降火湯[22], 眞武湯[23], 香砂養胃湯[24], 生肝健脾湯加味方[1], 十全大補湯[25], 理中湯[26] 등이 있었으며 대부분의 실험은 6-propyl-2-thiouracil (PTU) 투여 로 유발된 갑상선 기능저하증 모델 rat을 이용한 in vivo 연구였으며 모두 갑상선기능저하증에 대하여 유의한 효과를 보고하였다. 이에 반해 임상연구는 2018년도에 이뤄진 갑상선 질환에 대한 국내 증례 및 RCT 등 임상 문헌 체계적 연구를[27] 보면 증례보고가 6편, 대조군 연구가 2편, 무작위 대조 연구가 1편이었다. 특히 증례보고 6편은 모두 대상자가 1명이었다. 이처럼 아직은 한의학의 갑상선 기능저하증에 대한 임상연구는 부족한 실정이다. 이에 갑상선 호르몬제 복용없이 한방 단독 치료로 증상이 개선될 수 있는가, 혈액검사상 수치도 정상화될 수 있는가, 증상과 수치 모두 개선될 수 있다면 그것이 치료할 때 한시적인 효과가 아니라 지속될 수 있는가 라는 의문을 가지고 갑상선 기능저하증 진단을 받고 4년간 Levothyroxine을 복용하던 중 환자의 동의 하에 약물 복용을 중단하고 한의학적인 치료를 하여 유의미한 결과를 도출하였기에 보고하는 바이다. 덧말
